What Is Indiana’s PathWays Waiver? Health and Wellness Support for Adults with Disabilities

What Is the PathWays Health & Wellness Waiver?
The PathWays Waiver is Indiana Medicaid’s newest Home and Community-Based Services (HCBS) waiver, created specifically for adults with intellectual or developmental disabilities (I/DD) who don’t require the high level of support provided under other waivers like CIH (Community Integration and Habilitation).
This waiver focuses on wellness, independence, and meaningful community participation. It offers supports that help individuals live safely at home while building skills for daily life and self-advocacy.
Who Is Eligible?
To qualify for the PathWays Waiver in Indiana, an individual must:
- Be at least 18 years old
- Have a diagnosed intellectual or developmental disability
- Meet Medicaid financial and clinical eligibility
- Want to live outside of an institutional setting
- Not require 24/7 intensive support (CIH waiver would apply in those cases)
This waiver is often ideal for individuals who need moderate support to live independently, maintain wellness, or stay connected to their communities.
What Services Are Available Through the PathWays Waiver?
Under this waiver, participants can access a customized range of services designed to support both their health and independence. Services may include:
- Attendant Care – Help with personal hygiene, dressing, meals, and mobility
- Home and Community Assistance – Support with tasks like housekeeping, errands, and social interaction
- Structured Family Caregiving – Available in specific live-in family care situations
- Wellness Coordination – Assistance with managing chronic conditions, medications, or healthcare appointments
- Habilitation Services – Skill-building for personal care, communication, and decision-making
- Transportation – Help with getting to work, appointments, or community activities
All services are tailored to the individual’s needs through a person-centered care plan, developed in coordination with a case manager.
